You can get the Penny item with any purchase that is $10 or more before coupons. There is a Mystery Penny Item coupon in the Publix Sunday Ad every week. Some stores do not require this coupon, but many have changed and do require it now, so check with your store to find their policy.
I wish Publix would do a little better with having enough of the penny item. I went on Monday and had to take salad dressing (which I obviously did not need) and I was disappointed not to get my water. Sunday is not a good day for me to shop. Oh well, I really can't fault them much.
